Epidural Kateter Yer Değiştirmesi

Santral bloklardan olan epidural kateter uygulamalarının morbid obezlerde başarısızlık oranı yüksektir. Son yıllarda ultrasonografinin kullanım pratiğine dahil olması ile kateter uygulamalarında başarı oranı yükselmiştir. Kateterin başarılı yerleştirilmesiyle birlikte korunması da blok kalitesi için gereklidir. Morbid obezlerde cilt altı yumuşak doku kitlesinin artması kateterin yer değiştirmesine neden olmaktadır. Morbid obezlerin çok hızlı kilo vermesi sonrasında, ideal kiloda olsalar dahi cilt altı dokuların aşırı mobil olmasına bağlı olarak kateterin yer değiştirmesi sorununun hala devam ettiğini bu olgu sunumu ile vurgulamak istedik.

Displacement to Epidural Catheter

Epidural catheter applications as a central blocks have a high failure rate in the morbidly obese patients. Success rate has increased in recent years with the use of ultrasonography in practice for catheter applications. Successful placement and secure of the catheter is required to protect the quality of the blocks. Subcutaneous soft tissue mass in the morbidly obese patients is caused to the displacement of the catheter. After morbidly obese patients lose weight fast, catheter displacement still continued depending on excess mobile subcutaneous tissue. With the presentation of these cases, we focused on this problem.
