We present the case of a 77 year old patient with severe coronary heart disease, undergoing radical mastectomy with axillary lymph node dissection by ultrasound-guided continuous paravertebral block (CPVB).Radical mastectomy with axillary dissection is a surgical procedure that usually performed under general anesthesia and necessitates endotracheal intubation. Patients with coronary heart disease (CHD) and sleep apnea syndrome (SAS) have higher risk for general anesthesia. Ultrasound guided CPVB is a simple and safe alternative technique that offers anesthesia as well as postoperative analgesia with minimal side-effects.
