Adipose-Derived Stromal Vascular Fraction Cells Therapy in Hemophilic Arthropathy, A Case Report

Stromal vascular fraction (SVF) is a heterogeneous collection of cells obtained from adipose tissue through lipoaspiration and an alternative intraarticular treatment option especially in osteoarthritis (OA). The anti-inflammatory and extracellular tissue repair-stimulating properties of SVF increase its effectiveness in regeneration and repair mechanisms. One of the most common symptoms of hemophilia A and B is hemophilic arthropathy (HA). If HA is not adequately managed initially, patients may require major surgery, including total joint arthroplasty. In a 34-year-old male patient who was treated with intra-articular adipose derivated SVF with the diagnosis of hemophilia B and right grade 4 hemophilic knee arthrosis; clinical and radiological scores showed significant improvement in the 3rd month, 6th month and 1st year controls after the procedure. Intra-articular adipose derivated SVF treatment should be kept in mind as an effective treatment option with minimal invasiveness and few side effects in HA that does not respond to conservative treatments.

Stromal Vask�ler Fraksiyon H�creleri (SVF), adipoz dokudan lipoaspirasyon yoluyla elde edilen heterojen bir h�cre toplulu�udur ve �zellikle osteoartritte (OA) alternatif bir eklem i�i tedavi se�ene�idir. SVF'nin antiinflamatuar ve h�cre d�� doku onar�m�n� uyar�c� �zellikleri, rejenerasyon ve onar�m mekanizmalar�ndaki etkinli�ini artt�r�r. Hemofili A ve B'nin en yayg�n semptomlar�ndan biri hemofilik artropatidir (HA). HA ba�lang��ta yeterince iyi y�netilmezse total eklem artroplastisi gibi b�y�k cerrahi prosed�rler gerektirebilir. Hemofili B tan�l� ve sa� diz grade 4 hemofilik artrozu olan 34 ya��nda erkek hastaya eklem i�i adipoz t�revli SVF h�creleri uyguland�. ��lem �ncesi sonras� 3. ay, 6. ay ve 1. y�l kontrollerinde klinik ve radyolojik skorlar�nda anlaml� d�zelme g�zlendi. Konservatif tedavilere yan�t vermeyen HA'da eklem i�i adipoz t�revli SVF tedavisi minimal invaziv ve az yan etki ile etkin bir tedavi se�ene�i olarak ak�lda tutulmal�d�r.
