Intraperitoneal bupivacaine injection and infusion for postoperative pain management in laparoscopic cholecystectomy

Laparoscopic cholecystectomy causes three types of pain (visceral, parietal and shoulder pain). The purpose of this prospective study was to evaluate the effect of local analgesia via a subphrenic catheter on postoperative pain scores. Sixty patients (ASA I-II) who were undergoing elective cholecystectomy were studied after approval by the ethic committee. Local anesthetics or placebo solutions were given via a subphrenic catheter as follows: bolus injection immediately after the creation of pneumoperitoneum (S1), bolus injection at the end of the operation (S2) and infusion after the operation during postoperative period for four hours (S3). Patients were randomly divided into four groups: Group I (control) received 20 ml of % 0.9 saline during S1, S2, S3. Group II (pre-cholecystectomy) received 20 ml of % 0.5 bupivacaine during S1 and 20 ml of % 0.9 saline during S2, S3. Group III (post-cholecystectomy) received 20 ml of % 0.9 saline during S1, S3 and received 20 ml of % 0.5 bupivacaine only during S2. Group IV (infusion) received 20 ml of % 0.9 saline during S1, S2 and 20 ml of % 0.5 bupivacaine infusion during S3. The degree of postoperative pain was assessed using visual analog scale (VAS). Consumption of analgesic agents (i.v. or i.m. meperidine when VAS scores ? 4), the presense of shoulder pain, nausea and vomiting were recorded. Although VAS pain scores were less in all groups, it was only significant in Group IV when compared to the control group. The least analgesic requirement occurred also in Group IV

Laparoskopik kolesistektomi sonrası postoperatif ağrı tedavisinde intraperitoneal bupivakain enjeksiyonu ve infüzyonu

Laparoskopik kolesistektomi sonrası parietal, viseral ve omuz ağrısı olmak üzere üç tip postoperatif ağrı oluşmaktadır. Bu prospektif çalışmanın amacı bir subfrenik kateter yardımı ile oluşturulan lokal analjezinin sistemik analjeziye kıyasla postoperatif ağrıya etkisini araştırmaktır. Elektif kolesistektomi ameliyatı olacak (ASA I-II) 60 hastaya hastane etik komitesinin izni alındıktan sonra, lokal anestezik ve plasebo solusyonlar bir subfrenik kateter yardımı ile şu şekilde verildi: S1 pnömoperitoneum oluşturulduktan sonra bolus enjeksiyon, S2 operasyon sonunda bolus enjeksiyon, S3 operasyondan sonra postoperatif dönemde dört saat boyunca infüzyon. Hastalar randomize olarak dört gruba ayrıldı. Grup I’e (kontrol) S1, S2, S3’de 20 ml % 0.9 salin; Grup II’ye (pre-kolesistektomi) S1’de 20 ml % 0.5 bupivakain, S2 ve S3’de 20 ml % 0.9 salin; Grup III’e (post-kolesistektomi) S2’de 20 ml % 0.5 bupivakain, S1 ve S3’de 20 ml % 0.9 salin; Grup IV’e (infüzyon) S1 ve S2’de 20 ml % 0.9 salin, S3’de 20 ml % 0.5 bupivakain infüzyonu uygulandı. Hastaların postoperatif dönemdeki ağrıları vizüel analog skala (VAS) ile değerlendirildi. Kullanılan analjezik ajan miktarı (VAS ? 4 ise i.m. veya i.v. meperidin), omuz ağrısı, bulantı ve kusmanın varlığı kaydedildi. Postoperatif VAS ağrı skorları, kontrol grubuna göre bütün gruplarda daha az olmasına rağmen, sadece infüzyon grubunda istatistiksel olarak anlamlı farklılık bulundu. En az analjezik ihtiyacı da infüzyon grubunda oldu.
